Evan Applegate, PT, DPT
Evan is a born and raised Parowan native. He attended Southern Utah University for his undergraduate education (go T-birds!) and then earned his Doctorate of Physical Therapy at Midwestern University in Glendale, AZ. At Midwestern he supported his class as president and graduated with high honors. Evan has worked with a variety of ailments in outpatient orthopedics, long term skilled therapy and home health. He has also specialized in back and neck treatments. Evan always dreamed of returning to Southern Utah to open his own clinic. He wanted to serve the community he grew up in and he’s been livin’ the dream since January of 2019. Evan loves his work and outside of the clinic he enjoys reliving the glory days playing basketball, working in his garden, grilling up a mean burger and traveling/camping/hiking with his wife Chellese, four boys (he’s hoping to raise his own personal basketball team), and one girl. Evan thrives on understanding how the body works and is driven to restore and improve others lives through physical therapy.
Jon Hermansen, PT, DPT
Jon Hermansen is a Doctor of Physical Therapy who graduated from the University of Maryland, Baltimore in 2020. Jon was president of his class of 65 students, and has excelled in the classroom and clinical settings. His favorite part of being a physical therapist is interacting with his patients and making a difference in their lives. Jon is originally from Cedar City, Utah and enjoys competing in triathlons, rock climbing, backpacking, mountain unicycling, woodworking, and exploring the world with his wife and two daughters.
Jason Lovell, DPT
Jason Lovell is a Doctor of Physical Therapy who grew up in Delta UT, received his bachelors in exercise science and at SUU before earning his doctorate degree at Rocky Mountain University of Health Professions in April 2021. Jason enjoys working with people feels great satisfaction in developing friendships with patients and helping his friends get better. Coming from a rural community, Jason grew up with a love of life in a small community and spending time in nature while camping, hiking, hunting, fishing, rock climbing and riding ATV’s and motorcycles in the mountains of southern Utah with his family. Jason enjoys playing a variety of sports and loves to play guitar and sing with his 4 sisters.
Justin Davis is currently a student at SUU studying exercise science. He is originally from sunny St. George Utah! Justin enjoys snowboarding, playing the guitar, and rappelling down the beautiful canyons of southern Utah with his wife. He enjoys working at CCPT because of how much he gets to learn from his co-workers and all of the incredible patients!
Jennifer Doubek was born and raised in Parowan, Utah. She loves the small community and how everyone comes together in times of need. Jennifer enjoys working for CCPT, meeting everyone, and all of her co-workers. It’s a family of awesome people. Speaking of family, she has two children, son and daughter, that she loves watching grow into their own person. She enjoys time with family and being able to go support her nieces and nephews in their activities.
